Social Worker

You will work closely with the multidisciplinary team to deliver personalized support and plan for clients' discharges, while also liaising with home authorities and managing internal investigations. The role requires a Social Work degree, registration with Social Work England, and experience in a similar environment working with vulnerable adults.
