We are looking to recruit a highly skilled, enthusiastic individual to join us as a key member of our multi-disciplinary team working closely with Urgent Community Response to ensure that the physical and mental health needs of older adults are addressed. You will be a team player and work collaboratively with staff from all disciplines whilst maintaining high professional standards.

Providing urgent response to older adults who are experiencing a health crisis to support the most unwell individuals living in care homes, and this post provides a new and exciting opportunity to use your knowledge and expertise to reduce avoidable hospital admissions, minimise placement breakdown and reduce the use of anti-psychotic treatments. There is also the opportunity to shape service provision and personally develop.
