Full Job Description

We seek a dedicated and skilled Ageing Well Support Worker to join our new Proactive Ageing Well Service (PAWS) due to commence in October 2024. The purpose of the service is to proactively assess those with mild to moderate frailty, and create patient centred plans of care, ensuring the person is linked into the wider community MDT in order to improve symptoms of frailty, reduce disease burden and avoid unnecessary admissions to hospital.
The Ageing Well Support Worker will work across both clinical and administrative duties to support the Ageing Well Practitioners and service lead, by supporting with administrative duties, audit and data collection and visiting/telephoning patients to provide follow up and measure patient outcomes. You will also support with signposting to services and be involved in training activities with future potential to attend community events to promote ageing well awareness to older adults.
The post holder will be required to maintain excellent working relationships within the team and notify the practitioners immediately of any concerns related to patient progression, safety or wellbeing.
The post holder will be required to travel between and work at a variety of sites across Lewisham., To create Rio referrals, monitoring of service caseload, maintain an accurate Rio diary reflective of daily activity and documentation of patient outcome measures; carry out home visits and telephone consultations at defined intervals to measure outcomes of service interventions.
To provide information, teaching and instruction to patients, relatives, carers, other disciplines, and agencies regarding PAWS and service interventions; promote understanding on the aims of PAWS and to ensure onward referrals to appropriate community services.
Contribute to falls awareness training for relevant patient groups and to employ appropriate skills such as persuasion, motivation and negotiation to gain co-operation in the progression of agreed treatment programmes.
To complete onward patient referrals and signposting patients to other services as required and to maintain accurate clinical records of all interventions as per trust and legal guidelines.
Participate in multidisciplinary / multi-agency team meetings and case conferences to ensure the co-ordination of patient care.
Manage and prioritise own day to day workload and maintain level of service within the pathway at the local site and assist other members of the team to make appropriate prioritisation choices.
To participate in supervision and appraisal with designated practitioner; attend staff meetings; contribute to the clinical induction of team members and to be adherent to completing all mandatory training.

To collect data, statistics and information and input into computer systems as required
Deliver and install simple aids and equipment in the patient's home and instruct patients and carers in use of this equipment as appropriate.
To manage and prioritise own day to day workload and maintain level of service within the pathway at the local site and assist other members of the team to make appropriate prioritisation choices. To ensure the effective exchange of information across the pathway team and co-ordination of service delivery.
To participate the allocation of patient visits/telephone reviews.
To complete onward patient referrals and signposting patients to other services as required
To be professionally and legally responsible for own practice and accountable for all aspects of treatment delivered to the patient to ensure delivery of a high standard of clinical care.
To use effective communication, negotiation and empathy skills to maximise patient participation and adherence to rehabilitation programmes.
To be aware of limitations of own practice and seek advice and guidance from qualified staff as required.
To adhere to trust systems for incident reporting
To be proactive in supporting Practitioners with patient outcome measures.
To maintain comprehensive and accurate records (electronic and paper) and complete all mandatory documentation in line with legal and Trust standards in a timely and accurate manner.
To be compliant with all Trust Information Governance and confidentiality policies
To participate in service education and training rotas and be proactive in identifying own learning and development needs
Under the guidance of qualified therapists participate in service development projects and audits
To undertake any team administrative duties as required.

Financial
Ensure adequate activities of daily living equipment and other appropriate equipment is available and accessible, and work with Transferring Community Equipment Services (TCES) agenda.
Communications and Relationships
To communicate directly with staff regarding any relevant information for patient care
To provide information and instruction to other disciplines to ensure a consistent approach to patient care.
To educate patients / relatives/ carers regarding the nature of the condition and the aims of PAWS intervention.
To build relationships and promote PAWS both within the trust and to external organisations and partners.
General
To maintain accurate computer and written records that meet professional Trust standards.
To undertake general administrative tasks as required including filing, appointment booking, telephone triage, processing referrals, answering the telephone, and other duties as required.
To be able to work flexibly between sites, depending on demand, and be able to prioritise and organise own workload to facilitate this.
