Care Coordinator

We are now looking for someone with experience of working with children with a learning disability and/or Autism to take on the role of coordinator for our short breaks programme for children in Camden. As Camden Short Breaks Coordinator, you will be responsible for our delivery of person-centred home- based and community-based outreach support for children and young people living in the borough, ensuring that our service is of the highest quality and meets all CQC standards and contract requirements. This role involves line managing a team of support workers and positive behavioural support (PBS) workers, liaising with the local authority and local schools, working in partnership with family carers and children and young people to deliver a high-quality service . Above all, we are looking for someone who is an empathetic and caring communicator who can provide consistent and informed support to families in need. In addition , we would love you to be an innovative and organised team player who will contribute positively to the team. Savaas is really active in his local community and is involved in various clubs and activities. Savaas joined our weekly Wednesday garden project in August 2010 and has been a dedicated volunteer ever since.
