Full Job Description

The main duties of this role are to take calls from the MDT , family and other organisations. Liaise with patients and family and direct them them to the most appropriate team for their needs, utilising many additional services as well as general practice teams. On occasion will be asked to support the ward as well.

Working on a frailty ward with patients requiring support, It is important that the right applicant is able to communicate effectively at all levels and remains calm under pressure. Excellent organisational and IT skills are required.
