Full Job Description
To lead on the management and support of every aspect of a patient's pathway both internally to the organisation and externally (Independent sector and Mutual Aid NHS hospitals) ensuring the oversight of the effective and timely transfer of patients.
To support the General Manager for the RTT management of a specified surgical service(s) elective recovery improvement plan. The delivery of national waiting times targets is one of the Trust's key priorities, and the use of offsite elective capacity in an efficient way is critical to delivering these critical performance targets.
Successful relationship management and influencing skills are key to the role. The post-holder will have very significant patient contact (usually via telephone). They will be responsible for obtaining consent from patients to be treated at an offsite provider, give information as required, co-ordinate every aspect of the patient's offsite pathway, and address any concerns or complaints.
The post-holder will also need to work closely with Divisional Management Teams, Clinicians, and the Trust's corporate performance team. They will need to ensure accurate records of the pathway are kept on the patient information system at every step of the pathway and ensure Directorates have carried out their responsibilities in terms of reviewing patients, transferring notes, recording outcomes and organising patient follow-ups.
