Full Job Description

Through strong clinical leadership the Clinical Manager will provide operational and strategic day to day management of the Discharge Hub. She/he will lead, develop and professionally line manage a defined group within the Discharge Hub to maintain and improve standards of care and patient outcomes in all aspects relating to patient discharge. The post holder will lead in partnership with social care; jointly they will inspire and motivate the performance of their teams to ensure optimum patient care and service delivery. Working in partnership with social care staff the post holder will deliver corporate responsibilities across the whole of their portfolio, working collaboratively across organisational and professional boundaries, supporting the delivery of key objectives and strategic goals that drive quality assurance and improvement. She/he will promote best practice which inspires and motivates others, empowering them to ensure all staff, within their areas of responsibility, have the authority and support to provide agreed standards of care and service delivery to patients and their carers.Main duties of the job

  • Lead with credibility and authority assuming responsibility for the day-to-day operational management of the Trust's discharge hub.
  • To provide expert advice and leadership that ensures discharge plans are appropriate to ensure a safe, effective discharge and to minimise re-admission.
  • To build and establish good working relationships with key strategic partners across social and health care providers to facilitate patient discharge.
  • The post holder provides forward-thinking clinical and professional leadership to staff ensuring that discharge planning is delivered to a consistently high standard. She/he contributes to the on-going implementation of Trust wide practice and quality improvement. The post holder will undertake a key role in ensuring the Division and its services meet the quality, activity and financial targets through effective planning, delivery, service improvement and management of performance improvement.
