As a Community Development Co-ordinator, you will beresponsible for building and maintaining partnerships with local organisations,healthcare teams, and community groups to address the needs of deprived areasand bring people together. Your role will consist of partnership and engagementwith a network of local services, that support the wider determines of health,with a focus on reducing health inequalities in the areas that need it most. Workingcollaboratively with external stakeholders on new / current initiatives, with theaim of driving referrals into service. Thisposition is ideal for someone passionate about building relationships anddriving positive change at a community level. Trainingwill be given to ensure staff / applicants are equipped to deliver effectiveservices., This post will work under the line management of the Operations TeamLead . Your key responsibilities will be : Work closely with operational colleagues to manage referral rates anddevelop partnerships with all potential referrers within your locality. Organise and attend GP practice / PCN meetings to promote IntegratedWellbeing Services (IWS) and sustain referral pathways. Attend local healthforums, groups, and meetings regularly to represent ABL, promote IWS, andmaintain local partnerships. This will involve public speaking and presentingto small / medium groups. Support communications strategies, including public events, training andawareness sessions, and the creation of promotional materials ( socialmedia), to raise awareness of the service and capture referrals. Some weekendwork may be required to support these activities. Support all co-production activities with local residents andstakeholders to ensure the continuous development of the service. Collaborate closely with the entire delivery team within the Notts IWS. Support referral agencies by ensuring they provide appropriateinformation about the person being referred, including demographic data andwider determinants, such as caring status. Provide timely and relevant feedbackto referral agencies about those they referred. Work with individuals to co-produce simple, personalised support plansthat address their health and wellbeing needs, based on their priorities,interests, values, cultural and religious needs, and motivations. Create bespoke pilot programmes based on Notts IWS service requirements,identifying new opportunities to establish partnerships and managing orco-managing these projects. You will be a motivated, passionate, organised, and proactive individualwith significant experience in engagement. This role will be demanding as services develop and should becomeintegrated in all settings. It will be essential that you use your ownjudgement to prioritise competing demands and workload effectively to specifictimescales.

Experience incommunity health settings : Previousexperience within Community Development or similar community-based role isbeneficial. Adaptability toevolving services : Proven ability to work withinservices that are continually developing and changing. Understanding ofhealth inequalities : Knowledge of localdeprivation and the social, economic and environmental factors that impactpublic health.
