Full Job Description
Rethink Mental Illness is seeking a committed Care Navigator to provide tailored, high-quality support to individuals being released from HMP Millsike. As part of our integrated mental health team, you'll work closely with healthcare and prison staff to ensure that health services are in place to support a smooth, positive transition from prison to community life. Using a strengths-based, coaching approach, you'll offer one-to-one and group interventions, helping individuals manage their own wellbeing and reconnect with essential community resources.,
- Deliver pre- and post-release support to individuals with health vulnerabilities, assisting with continuity of care and connecting them to community services.
- Conduct initial assessments and care mapping for service users, creating tailored care plans for effective release and reintegration.
- Organise and contribute to health promotion events, supporting the reduction of health inequalities and the promotion of wellbeing.

NVQ Level 3 or equivalent in Health and Social Care or related field. - Experience supporting vulnerable individuals to set and achieve goals, with knowledge of health, criminal justice, housing, and social care.
- Strong communication skills with experience in case management, structured interventions, and care planning.
- Proficiency in Microsoft Word, Excel, and database systems, along with effective time management and problem-solving abilities. Desirable: Experience within a prison setting or working with individuals involved in the criminal justice system, and additional mental health or coaching training.
- Appointments will be at the minimum of the band. We do reserve the right to close this advertisement early if we receive a high volume of suitable applications.
- Given the nature of this role, which involves frequent travel to various locations and timely meetings, a clean drivers licence is essential.
