Full Job Description
The post holder will work within the Adult Mental Health Team providing social care expertise and care coordination.
The post holder will ensure that all service users with the AMHT have full assessment of their Care and Support needs under the Care Act 2014 and that support planning takes place to meet these needs.
The post holder will identify and address any barriers to promoting a social model of mental disorder, in partnership with Service User, their family and support network and the Multi-Disciplinary AMHT.
The post holder will provide expertise for inpatient staff with regards to Adult and Children's Safeguarding responsibilities.
The post holder will provide expertise with regards to application of the Mental Capacity Act and Mental Health Act.
The post holder will demonstrate the skills and aptitude to maintain their Social Work professional identity when working within a Multi-Disciplinary team, 1.To work independently as part of a multi-disciplinary team to: assess, plan and implement patient specific programmes of care/interventions within the Care Programme Approach, contributing to the continuous holistic assessment of patients.
2. To participate in and contribute to the multi-disciplinary team, articulating the social care perspective and ensuring that meeting the social care needs of service users retains a high priority and profile within the functioning of the team.
3. To undertake Care Act assessments some of which will be for patients presenting with highly complex needs using your profession specific knowledge and skills.
4. Ensure that there are no delays to service users receiving appropriate care and support by completing the relevant process to access care funding and implementation of support plans.
5. To function effectively across a variety of environments (e.g. AMHT, ward, department, service users homes etc.) involving changing and demanding conditions, + Demonstrates the skills of managing care effectively in conjunction with the Multidisciplinary Team taking account of each member's contribution to the care package.
+ Demonstrates the skills of effective time management and ability to prioritise competing demands.
+ Able to work positively with clients or carers who express verbal concerns or complaints about the service and attempt early and effective resolution in conjunction with the PALS Service.

Is cognisant of local Trust policies and adheres to policy and procedural guidance.

Shows awareness of the major national policy initiatives and their influence on care delivery.

Able to assist the Clinical Leads and Team Managers in the management of change, taking account of the views of clients, carers, the ward and community team.

Actively works with the Team Leads and Team Managers to implement the vision for recovery focused community mental health care
+
Actively engages in the audit process and local audit projects.
+
Shows willingness to participate in the service evaluation.
+
Acts as a role model in the clinical arena for other staff.
+
Demonstrates the knowledge and skills necessary to provide effective clinical supervision of the shift co-ordinators and junior staff on the team.

Actively involved in Service Improvement initiatives within the area of work and as required by the mental health division.
